تأثیر تمرینات ورزشی و مصرف کورکومین بر فریتین‌سرم و آلبومین‌سرم در بیماران همودیالیز

چکیده

 
هدف: با توجه به تاثیر شاخص‌های التهابی بر مرگ و میر و اهمیت کاهش آنها، هدف پژوهش حاضر بررسی تأثیر تمرینات ورزشی و مصرف مکمّل کورکومین بر برخی شاخص‌های التهابی در بیماران همودیالیز است.
روش‌: در این مطالعه نیمه‌تجربی یک سوکور، با طرح پیش‌آزمون- پس‌آزمون با گروه کنترل ٦٨ شرکت‌کننده با بیش از ٣ ماه سابقه دیالیز شرکت کردند، این افراد به طور تصادفی به ٤ گروه 17 نفری تمرین، کورکومین، تمرین و مصرف مکمّل کورکومین و کنترل تقسیم شدند. مداخله ورزشی به صورت ١٢ هفته تمرین و مداخله مکمّل به صورت ١٠٠٠ میلی‌‌گرم کورکومین انجام گرفت. داده‌ها با استفاده از آزمون کولموگروف- اسمیرنوف و آنالیز کواریانس در سطح معناداری کمتر از 05/0 تحلیل شدند.
یافته‌ها: تاثیر توأمان تمرین و مصرف کورکومین باعث تغییرات معناداری به صورت کاهش میزان فریتین سرم و افزایش سطح آلبومین سرم و ظرفیت عملکردی می‌شود (P < 0.05). همچنین در گروه تمرین نیز نتیجه مشابهی مشاهده شد. مصرف کورکومین نیز به‌تنهایی سبب کاهش میزان فریتین سرم و افزایش سطح آلبومین سرم شد (P < 0.05)، اما افزایش معناداری در ظرفیت عملکردی مشاهده نشد (P > 0.05). بیشترین کاهش در سطح فریتین سرم (5/5±59-) و بیشترین افزایش در میزان آلبومین سرم (25±811) و ظرفیت عملکردی (61/15±67) در گروه تمرین همراه با مصرف مکمّل کورکومین مشاهده شد.
نتیجه‌گیری: تمرینات ورزشی به همراه مصرف مکمّل کورکومین باعث کاهش التهاب بیماران دیالیزی می‌شود و میزان تاثیر‌گذاری آن نسبت به تمرین به‌تنهایی و مصرف کورکومین به‌تنهایی بیشتر است.

The Effects of Exercise Training and Curcumin Consumption on Serum Ferritin and Serum Albumin Levels in Hemodialysis Patients

Objective: The objective of this study was to investigate the impact of exercise training
and curcumin supplementation on inflammatory indices in hemodialysis patients, considering the relationship between these indices and mortality, as well as the importance of reducing them.
Method: In this semi-experimental, single-blind, pre-test/post-test design with a control group, 68 participants with over three months of dialysis history were involved. These individuals
were randomly divided into four groups of 17: exercise, curcumin, exercise with curcumin supplementation, and control. The exercise intervention consisted of 12 weeks of physical activity, while the supplementation involved 1,000 mg of curcumin. The data were analyzed using the Kolmogorov-Smirnov test and analysis of covariance, with a significance level set at less than 0.05.
Findings: The combined effect of exercise and curcumin consumption resulted in significant changes, including a decrease in serum ferritin and an increase in serum albumin and functional capacity (P<0.05). A similar result was observed in the exercise group. Curcumin consumption alone led to a decrease in serum ferritin and an increase in serum albumin (P<0.05); however, no significant improvement in functional capacity was noted (P>0.05). The most significant decrease in serum ferritin (-59±5.5) and the most notable increase in serum albumin (811±25) and functional capacity (67±15.61) were observed in the exercise group receiving curcumin supplementation.
Conclusion: Exercise training combined with curcumin supplementation reduces inflammation in dialysis patients, demonstrating greater effectiveness than either exercise alone or curcumin alone.
