The principles of prehabilitation are to reverse risk factors that expose individuals to worse outcomes during stress on the body. It predicts the impact of surgery on postoperative performance to better reduce the negative consequences of surgery. In addition, clinicians should address medical issues such as components of metabolic syndrome that may negatively impact response to and recovery from various surgeries and cancer treatments. Prerehabilitation is ultimately intended to improve outcomes, prevent disease recurrence, and maintain function. The purpose of this policy brief is to educate clinicians on the basic concepts of prerehabilitation, including its importance for certain populations, the benefits of various exercise programs, nutritional considerations, and dietary/supplementary strategies. These described populations often fall into two broad categories, such as those with multiple metabolic disease comorbidities such as obesity, diabetes, and metabolically related fatty liver disease/increased visceral fat burden, and those with sarcopenia and frailty. Exercise programs and nutritional considerations describe strategies to build/maintain skeletal muscle and, when appropriate, various approaches to caloric restriction to reverse elements of metabolic disease.
